All new clients to it’s time! must fill out a PAR-Q readiness for exercise questionnaire, as well as a gym waiver. You might also want to fill out some of our other forms ahead of time, depending on your condition, goals, and needs.

PAR MED X - PHYSICAL ACTIVITY READINESS MEDICAL EXAMINATION

If you answered “yes” to any of the follow-up questions on pages 2 and 3 of the PAR-Q, you and your physician should fill out a PAR MED X form.

DAILY FOOD AND MINDSET JOURNAL

If you would like to have a Precision Nutrition consultation, the Daily Food and Mindset Journal should be filled out for three separate days and submitted to the front desk at least a week prior to your consultation appointment. This allows our Precision Nutrition specialist to analyze the forms ahead of time, and we can make the most of your consultation time.
